Tax Member IRS Form
A few years ago the IRS came out with a new form to identify
which member of an investment club was the "Tax Member".
The Tax Member is the member the IRS will contact if it has
any questions about the club. I am in the unbelievable
position of training a replacement. Does anyone know the
IRS form number for reporting a new Tax Member?

This is the form you should use to update the "responsible party" for your club with the IRS:

Form 8822-B

Laurie,

I have been doing my club's taxes for over 15 years and never submitted this form . I just fill in my name on the tax submission and my name and address is listed as the home address for our club. Should I send in this form?

No. They already have received the information they need based on what you have sent. It would only be if you change treasurers going forward.
