Taxes/form 7004 of course
Ok, I was really hoping I would get through all this without having to ask any questions, but now I am afraid I messed up my 7004 form. 

I put in club name, address, id #, etc.   On line 5a I put 21 for calendar year 2021. But I did not put any zero's on line 6, I left it blank. 

What I did do, because I probably read into it too much is towards the top where is says 'application for' and it has the list of all the tax types 
I put 09 thinking it was asking for the code and that is what we file.   So now I am afraid my application for an extension is going to be rejected. 
Should I send in a 'revised' 7004?   I know there were a number of you stating 'read the directions' 'it's not that hard'  so sorry about this.

Don't worry about it. The IRS will accept the application with lines 6-8 blank. They really apply to some of the other entities that use this form to file for an extension.
